Health Card Terms and Conditions - Family Medical Center (FMC)
By applying for or using the FMC health card, you agree to the following terms and conditions:
Card Expiration
- The health card will be valid for one (1) year from the date of issue.
- Renewal is required upon expiration to continue services.
Subscription and Monthly Payment
- Charges will be applied according to the selected monthly subscription package of the esteemed users.
- Payment must be made in advance, and services will be suspended or terminated in case of non-payment.
- No refunds will be issued for partially or unused services.
Eligibility and Coverage
- This card is valid for the following individuals:
- Primary cardholder (husband/wife)
- Dependent children under eighteen years of age (verification of dependency may be required).
Scope of Services and Limitations
- Family Medical Center home care services will be available only within five kilometers.
- Additional charges will apply for distances beyond this limit.
- Emergency services, specialized treatments, or procedures not included in the subscription will incur extra charges.
Cancellation and Termination
- The card will be canceled in case of violation of terms and conditions.
- The card will also be canceled if there is misuse or fraud.
- Customers may cancel their subscription with a written notice 30 days in advance; no partial refunds will be issued.
General Conditions
- This card is non-transferable and will be verified at the time of service.
- Family Medical Center reserves the right to change terms, prices, or services with prior notice.
- Lost or stolen cards must be reported immediately; a fee may apply for a replacement card.
Governing Law
- These terms will be governed by the laws of Islamic Republic of Pakistan and any disputes will be resolved in its courts. By using the health card, you acknowledge that you have read, understood, and agree to these terms.
